NLM Georgia Biomedical Informatics Course: For Current Participants

Submission Deadline: March 1, 2017

• Complete the Participant Questionnaire.  
• Email three PowerPoint slides with your photo, brief bio, and something surprising someone would never guess about you to gabiomed@augusta.edu.
• Fill out a Service Agreement Request (SAR) form and a W9 (forms located below).  Email completed forms to gabiomed@augusta.edu . 
• Register for your CE credit (except MLA CE) through Augusta University’s Continuing Education Department.

Submission Deadline: August 7, 2017

• This is the deadline for travel arrangements to be completed.
• If you are flying, you will be REQUIRED to make travel arrangements through Patty Font at Augusta Travel, 706-737-2308. Shuttle service from the Atlanta, GA airport to Brasstown Valley will be provided. Shuttles will leave Atlanta on September 10, 2017 traveling to Brasstown Valley at approximately 1:00 PM and 3:00 PM.  (See Travel Guidelines below.)
• Submit activity selection  
• Submit meal selections
• Log on to Brightspace/Desire2Learn to review content and instructions (You will receive an email for this in August.)
